When you are killing monsters, they randomly drop this item:

This can be dropped from any monster.
Sell it to an npc, and it will randomly give you gold.
The gold you recieve can be 1 Gold, or even 500.000.000 Gold.
Depends on your luck.

You can get the Custom Titles from this npc in the Newbie town.
Click on him and you can see the missions.
Press Q and you can see what you need to Complete the missions.
When you are done, go back to the npc and finish the quest. Your title will be changed automatically.

Sell it to an NPC of your Choice - Doesnt matter in my Opinion which 1 you use.
And dont forget its like Alchemy - Luck Based how much Gold you will gain!

I just bought a Title Item. Apparently it's a quest item but I can't seem to figure out how it really works. Do you know anything about it by chance?

Title Items are to change your Title. There is no other Reward.
To complete an Title Quest you need atleast 50 Title Items (50 Silks). Then speak to the same NPC where you bought it and talk with him. You will see a huge list of different Titles - Choose one.

Hmm I do not see any new options. I did buy 50 of them now and nothing. I did read in a diff topic it might have to do with my lvl? I'm lvl 114 atm, so perhaps I'm still too low to get one of those titles?

Yeah try to get 120+ or 125+ then it should be working.
Im 130 and can see all. Good luck with that

Actually Jelly Lottery can give you from 1gold to 2b ! I got 2 times 2b so that's why I know....

I Didn't said 500mil is the max that it gives.
